COVID Waiver for Activities
The district has the following waiver they would like completed, signed, and turned in upon the student's first day in an activity. Please review, print, complete and send with your student on the first day. We will have copies available if a student needs to pick one up on their first day to take out and have completed. Please let us know if you have any questions or concerns.
